29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails. Inside roof access. On or about March 6, 2013, the fixed ladder used to access the roof through the open hatchway did not have side rails (or equivalent protection) that extended 3 1/2 feet above the roof landing level for employees to hold onto to prevent falls to the floor below when climbing from the ladder onto the roof. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
