29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: 422 Pike Place, Windsor, CT 06095 on or about 05/14/2013: The employee was utilizing an extension ladder and the side rails of the did not extended at least 3 feet above the upper landing surface exposing the employee to the fall hazard. All Phase Renovations, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 311798649, citation number 01, item number 003 and was affirmed as a final order on 05/05/2010, with respect to a workplace located at 914-928 Hopmeadow Street, Simsbury, CT 06070.
