29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program. Information in the training program was not updated to be consistent with changes in protective equipment and work processes: a) Production Area : On or about May 1st, 2013 the employer exposed employee(s) to noise at levels of 108.2% (equivalent to an 8 hour time weighted average TWA of 90.5685 dBA) which is 2.16 times the Action Level of 50 % (equivalent to an 8 hour TWA of 85 dBA) while sorting in the production area. Results were calculated for a 421 minute sampling period excluding a 30 minute lunch. The employer failed to provide training. b) Combination Pallet Station : On or about May 1st, 2013 the employer exposed employee(s) to noise at levels of 237.4 % (equivalent to an 8 hour time weighted average TWA of 96.23 dBA) which is 2.374 times the Permissible Exposure Limit of 100 % (equivalent to an 8 hour TWA of 90 dBA) while operating a nail gun at the combination pallet station. Results were calculated for a 425 minute sampling period excluding a 30 minute lunch. The employer failed to provide training. c) Trim Saw Operator : On or about May 1st, 2013 the employer exposed employee(s) to noise at levels of 121% (equivalent to an 8 hour time weighted average TWA of 91.3810dBA) which is 2.42 times the Action Level of 50 % (equivalent to an 8 hour TWA of 85 dBA) while operating the trim saw. Results were calculated for a 393 minute sampling period excluding a 30 minute lunch. The employer failed to provide training.
