29 CFR 1910.38(f): An employer did not review the emergency action plan with each employee covered by the plan. Employee(s) had not received appropriate training on the actions necessary for a chemical release from stored chemicals located in the K-Pack area of the facility. April 14, 2013: Maintenance employee(s) needed treatment at a local hospital from chlorine gas exposure that occurred when they exited through the K-Pack area, rather than an alternative egress route. An accidental mixing of sodium hypochlorite bleach and lactic acid caused a chlorine gas release from a 2,000 gallon tank in the K-Pack area.
