29 CFR 1910.269(d)(2)(viii)(A): Retraining was not provided for all authorized and affected employees whenever there was a change in their job assignments, a change in machines, equipment, or processes that present a new hazard or whenever there was a change in the energy control procedures: At the AMEREN-UE Calloway Nuclear Plant, switchyard, specifically at Bus B, 52-4, an arc flash occurred on or about 04/02/13 severely burning employees. The employees, normally assigned to work with electrical equipment rated at 480 volts or less, were tasked with grounding Bus B, 52-4, rated at 13.8 kv.
