29 CFR 1910.119(f)(1)(i)(D): The employer did not develop and implement written operating procedures that provide clear instructions for emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner: (a) Fort Beeler Facility, Cameron, WV: Written procedures for conducting a manual emergency shutdown of the plant and the conditions requiring a manual emergency shutdown were not developed for operators running the TXP-1, TXP-2, and TXP-3 processes that contain greater than 10,000 pounds of flammable liquids and gases as determined on March 28, 2013.
