29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: A) 492 10th Street, Fort Lee - NJ: An employee was exposed to fall hazards of approximately 20 feet while using an extension ladder to access a pump jack scaffold. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about January 8th, 2013.
