Text For Citation: 01 Item/Group: 002 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) On or about January 19, 2013, for employees engaged in well servicing operations including but not limited to fracturing operations where the employer failed to control the release of stored energy from an open-end, bleed-off valve of a discharge line (pressure line), resulting in struck-by hazards from the uncontrolled movement at the Kodiak oil site, KOALA 154-97-15-34-27-1H3, Watford City, ND. Abatement Note: One feasible and acceptable abatement method to correct the hazard would be for the employer to ensure personnel perform lockout/tagout procedures on the bleed-off valve of the discharge line (pressure line) to control the energy in accordance with (API) American Petroleum Institute Recommended Practice 54, Third Edition, "Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations", section 6.9, LOCKOUT/TAGOUT, subsection 6.9.1b which states: "Personnel should be trained and disciplined in the use of this system to prevent unexpected operation of any equipment that stores any type of energy that might inflict injury to personnel."
