29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: Yard - A four leg chain sling was secured to the fork of a Taylor model TE-155S fork truck to lift and transport concrete jersey barriers. The employer did not obtain the lift truck manufacturer's written approval, and a capacity data plate was not provided as necessary.
