29 CFR 1910.305(g)(2)(ii) Flexible cords were not used in continuous lengths or without splices or taps. It was found on October 12, 2012, the employer had attempted to correct a reverse polarity wiring of their Smart Box 3 equipment by splicing the flexible cord used to power the equipment, reversing the conductors to eliminate the reversed polarity, then securing the conductors together with electrical wing nuts, then using red electrical tape to insulate the wiring.
