29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: (a) On or about October 22, 2012, for the employees exposed to fire and explosion hazards where ignition sources, including but not limited to spark producing tools and battery operated tools, were not controlled when flammable vapors were created from the rupture of a crude oil tank that released approximately 7995 gallons at the Nathan Hale 4-25H NW 25-150-94 well site, Mandaree, ND.
