29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by a crushed by hazard; a) On or about 10/23/2012 at 654 Univeter Road Canton, GA; the employer failed to ensure that the space between the #1 Hopper and #1 Dumper was guarded to keep employees from entering the areas of operation. b) On or about 10/24/2012 at 654 Univeter Road Canton GA; employees were exposed to an unguarded 3" diameter rotating collar that was not smooth with six exposed bolt heads. The shaft collar was in operation with the decline belt on the wing pack out line.
