29 CFR 1926.1419(e): The signals used (hand, voice, audible, or new), and the means of transmitting the signals to the operator (such as direct line of sight, video, radio, etc.), were not suitable for the site conditions. On or about July 5, 2012 the employer did not ensure that all persons involved in conducting a multiple crane lift over Lake Butte des Morts, Bridge B-70-266 Span 5, were able to communicate effectively with each other. The American 9299 crane was positioned such that the operator could not communicate with the operator of the Terex HC110. Employees were thereby exposed to the hazards associated with crane and load failures.
