29 CFR 1926.502(i)(4): All covers shall be color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard. a) First Level Rental Building #4 - While attaching clamps to a concrete wall frame, employee fell 17 feet to the basement level through a 2'x 6' hole in which the cover over the hole was not marked as such on or about 06/20/2012.
