29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Note: Specify distances as indicated in table in paragraph (f)(6) of this section. At the job site located at 51 Eager Road, Webster Groves, on 5 June 2012, the employer did not ensure the ladder jack scaffold was placed far enough away from the energized power line or that the power line had been deenergized or covered and insulated to prevent accidental contact. As a result the employee used the scaffold while exposed to the energized electrical power line. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
