29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) On the front of the building, on June 7, 2012, the employer did not ensure the use of an acceptable means of access and egress to and from a working platform, of a tubular welded scaffolding system, measuring seven feet from the ground level, exposing employees to the hazards associated with sprains, bruises, and lacerations. The employees were using an unstable, unbraced platform with improper footing with a step up to the working platform that was greater than two feet. Victor Mena was previously cited for a violation of this occupation and safety health standard or its equivalent standard 1926.451(e)(1), which is contained in OSHA inspection 314095563, citation number 1, item number 3 and was affirmed as final order on February 22, 2012, with respect to a workplace located 219 Mallard Loop Drive, Savannah, Georgia 31405. ABATEMENT CERTIFICATION IS REQUIRED
