29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about May 16, 2012, employees were upgrading equipment on a cell phone tower approximately 104 feet high by hanging under the work platform and using fall protection that was engaged directly to a rope without receiving fall protection training from the employer, exposing the employees to a fall hazard.
