29 CFR 1910.146(k)(3)(ii): An authorized entrant entered a permit space and did not have the other end of the retrieval line attached to a mechanical device or fixed point outside the permit space in such a manner that rescue could begin as soon as possible. A): Rescar Companies - On or about 5/11/12, an employee had entered a rail tank car (#GATX31606) to perform work. The employee was wearing a harness but the lifeline was not attached to a fixed point outside of the confined space.
