29 CFR 1926.21(b)(6)(i): All employees required to enter into confined or enclosed spaces shall be instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required. The employer shall comply with any specific regulations that apply to work in dangerous or potentially dangerous areas. a) The employees required to enter a crawl space of a residential house was not instructed on the potential of energized parts and service equipment such as the water service lines, and the metal HVAC ducting that were within the employees work zone. The electrical service was not de-energized or locked out before employees entered into an area of the crawl space and when a broken water line increased the electric shock potential in and around the employees work zone.
