Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: On or about November 15, 2011, at Village Creek SP GU #2 well site, employees were required to use a wire rope for Geronimo line that was kinked in the middle section of the line. This condition exposed employees to a fall hazard. Among other methods, one feasible and acceptable method to correct this hazardous condition is to ensure that the wire rope used as an escape line is removed from service when evidence of kinking, crushing, cutting, cold working, or bird-caging is observed, in accordance with the American Petroleum Institute Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations (API 54 9.6.3).
