Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about August 2, 2011, at the jobsite located at 65 Garfield in Clarendon, Arkansas an employee was observed operating an Eagle Model TE 300 forklift to transport lumber across the property and the employee was not wearing a seatbelt. The seatbelt had slipped beneath the seat bracket while the forklift was down for repairs. The maintenance worker inadvertently bolted the bracket down on top of the belt, trapping the buckle under the bracket. This practice exposed the employee to fall and crushing hazards in the event of a roll over or ejection.
