29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by nip points and/or points of operation: On or about February 17, 2011, an employee experienced a fatal crushing to the midsection of the body when he was caught between the closing door of the vacuum tank and the tank itself. No form of guarding or other means to prevent the crushing hazard was present.
