Text For Citation: 01 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment or a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: On or about July 26, 2011, employee was exposed to fall hazards where the locking snap- hook that was part of the personal fall arrest systems (PFAS) using by employee working at the derrick board was not been inspected before be used. American Petroleum Institute Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations (API 54 5.1.1) states that personal fall arrest systems (PFAS) shall be inspected prior to each use. Among other methods, one feasible and acceptable method to correct this hazardous condition would be to regularly inspect the fall arrest system before been used and replace any parts as necessary.
