29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in startup following a turnaround or after an emergency shutdown: 1. ETBE unit; the startup procedures for furnaces including, but not limited to, furnace F6101 did not address control board operator verification of pilot on the DSC prior to opening main gas valve.
