29 CFR 1910.119(d)(3)(i)(B): The Process Safety Information (PSI) pertaining to the equipment in the process did not include piping and instrumentation diagrams (P&ID'S): a) On or about February 24, 2010 - the PSI contained two flow diagrams with varying degrees of specificity about the ammonia refrigeration system. Neither flow diagram contained the piping, equipment nor instrumentation specifics that were required to identify, evaluate and control the hazards associated with the refrigeration process.
