29 CFR 1910.119(e)(3)(i): The employer did not ensure that process hazard analysis included the identification of all previous incidents which had a likely potential for catastrophic consequences in the workplace: 21178 SW 89th Rd., Beatrice, NE 68310 (Jobsite): The employer did not ensure that process hazard analysis included the identification of all previous incidents which had a likely potential for catastrophic consequences in the workplace. On January 9, 2008, five (5) contract employees were hospitalized after a lifeline raised a relief vent plate, exposing them to ammonia. This information was not included in the most recent process hazard analysis (PHA), which was completed in 2009. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
