Text For Citation: 01 Item/Group: 002 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Plant Maintenance Services, 720 N. FM 1611, Snyder, Texas. For the period of time up to and including June 8, 2010, the employer did not ensure that employees were trained in safe operation of vacuum trucks and equipment. One employee was seriously injured while operating a vacuum truck to clean desiccant from a gas dehydrator vessel. This condition exposed employees to fire/explosion hazards. Feasible and Acceptable Abatement Methods would include: 1. Ensure that employees who operate vacuum trucks in petroleum facilities are trained in safe operation of the vacuum truck and equipment. 2. Ensure that employee inspect and maintain the vacuum truck and equipment. 3. Establish and implement a written procedure in safe operation of vacuum trucks and equipment. Note: Abatement certification and supporting documentation are required for this item.
