Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an asphyxiation hazard: a) On or about 12/2/09 in the ammonia refrigeration room; two pressure Hansen brand relief valves ( serial # 128, Cat. No. H5 502, set pressure 250 psi. size 3/4" inlet and 1" outlet ) were protecting a pressure vessel with a maximum allowable working pressure of 300 psi. on low stage ammonia refrigeration compressor #7 were installed on January 2004 and due for replacement on January 2009. ABATEMENT DOCUMENTATION REQUIRED a) Feasible and acceptable abatement methods to correct this condition would be; conduct visual inspection at least every six months and at least every five years remove and replace with new or with overhauled and re-calibrated valves ( or cartridges). Reference; IIAR ( International Institute of Ammonia Refrigeration ) Bulletin No.110 - 3/93, Guidelines for: Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems; Section 6.6.3 " At least every five years pressure relief valves ( or cartridges) shall be removed and replaced with new or with overhauled and re- calibrated valves ( or cartridges)."
