Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed-by: a) Hand layout area- On or about 10/26/10- An overhead carrier beam rail in house fabricated and welded that was used to lift molds containing resin based materials weighing approximately 3,000 lbs was not tested, rated or inspected. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1. Mark the load capacity on the metal beam rail in accordance with ANSI B30.11-1980, Monorail Systems and Under Hung Cranes, Section 11.1 Construction and Installation. 2.) Inspect and test the metal beam rail in accordance with ANSI B30.11-1980, Monorail Systems and Under Hung Cranes, Section 11.2 Inspection, Testing, and Maintenance. ABATEMENT DOCUMENTATION AND CERTIFICATION IS REQUIRED FOR THIS ITEM
