Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injury from an improperly secured forklift attachment: a) Double J Meat Packing, Inc., 726 W. Main Street, Pierce, CO: On or about 4/13/2010, an employee was observed operating a Case forklift with an improperly secured self-dumping hopper. The self-dumping hopper was not attached to the fork truck to prevent it from sliding off the tines. Abatement Note: One feasible and acceptable means to correct this hazard is to follow page one of the Vestil owners manual that states the following: "Ensure that the dumper is locked in place and secured to the fork truck's fork mast before transporting it or raising it off the ground.
