29 CFR 1910.119(e)(3)(vi): The employer did not address all human factors related to closing specific isolation valves during emergency situations when it conducted its PHA: The employer does not consider human factors in that the employer did not identify and evaluate the locations of DP-70 emergency isolation values/panels in the H-Oil unit that could potentially place employees in a hazardous situation during emergencies, exposing employees to fire and/or explosion. On or about May 05, 2010, and July 20, 2010, this violation was found at Motiva Enterprises LLC, Convent, LA where the following DP-70 valves/panels were observed throughout the H-Oil unit: -LCB 101 (Train 100 Emergency Isolation Valves) TRN-100 DP-70 -LCB 102 (DGA Pumps, C-101 Comp, P-130 & P-180 -LCB 103 (P-117, P-101, P-151 & P-201 -LCB 201 (Train 100 Emergency Isolation Valves) TRN-200 DP-70 -LCB 301 (Diesel product pump suction -LCB 302 (Diesel product pump suction -LCB 401 (EM Dump Valves, M/U Comp Pursuant to 29 CFR 1903.19 within 10 calendar days, the employer must submit an abatement plan describing the actions it is taking to ensure that locations of emergency isolation valves/panels (DP-70s) that must be closed in emergencies are identified and evaluated. The violation must be abated no later than 30 days from receipt of this citation.
