29 CFR 1910.119(d)(2)(i)(D): Process safety information pertaining to the techno logy of the process did not include the safe upper and lower limits for such ite ms as temperatures, pressures, flows or compositions: This violation was observe d on or about 01/19/2010, where the employer did not maintain a listing of the safe upper and lower limits for the following safety d evice: a)Pressure safety valves (PSV's), Pressure Sensor High & Low (PSH & PSL) b)Flow safety valves (FSV's) c)Temperature Sensor High & Low (TSH & TSL) d)Level Sensor High & Low (LSH & LSL) These safety devices are located on, but are not limited to, the High & Low pressure Separators (MBD-0500 & -0600), Oil Heater/Tr eaters (NBK-0100 & -0200), Line Heater (BAP-0050), Vent Scrubber (MBF-0100) and Water Skimmer (MBM-0100). By not developing, obtaining, documenting the above in formation, the employees were exposed to fire, explosion and chemical exposure. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the citati on, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that the Hi gh and Low limits are documented on the Safety Analysis Function Evaluation (SAFE) chart, PSV Inspecti on reports, and other process documentation.
