Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of: a) On or about 01/26/10, at the work place, employee(s) were exposed to the hazards of being struck-by or crushed, as the Rotary lift did not have the anchor bolts tighten to the manufactures recommendations of 90 ft/lbs. Among other methods, one feasible and acceptable abatement is to always keep bolts tight. Check periodically according to the manufactures maintenance instructions, also every 3 months; check anchor bolts for tightness and torqued to 90 ft/lbs.
