Text For Citation: 01 Item/Group: 002 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between / amputation hazards, while performing fabrication work with a press brake: On or about 01/08/09 at Winder, GA: Employees were exposed to caught between / amputation hazards, while operating a press brake: a. The employer failed to develop die setting procedures for the two press brakes utilized in Building 824. b. The employer failed to guard the back of the Accurpress press brake located in Building 824. c. The employer failed to establish records for periodic and regular inspections and maintenance of press brakes located in Building 824. d. The employer failed to properly maintain training records for press brake operators. e. The employer failed to properly manage press brake mode selectors on press brakes located in Building 824. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS IS: 1. The employer should enforce the requirements for the use of press brakes at his workplace, as written in ANSI B11.3-1982(R1994). The employer shall establish a press brake safety program to include the training of employees, point of operation guarding, die setting procedures, inspection/maintenance records management, and the control of mode selector keys. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
