Text For Citation: 01 Item/Group: 002 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about October 1, 2008, at the job site located at 492 Highway 25, North, Guy, Arkansas, the employer did not use an approved lifting attachment when lifting a free rigged bar joist via a sling placed over the tines of a Skytrak 8042 for a below the tines lift, exposing employees to the hazards of being struck by the load in the event it slipped off the forks. Among other methods, one feasible and acceptable abatement method to correct this hazard is to (1) ensure that an approved lifting hook or other attachment approved by the manufacturer is used when lifting a load and (2) use the pins provided on the attachment to secure the lifting hook behind the heel of each fork to protect against the attachment being disengaged from the forks.
