this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employee operating a Lull forklift, Model 644TT34, with the operator's seat not attached to the forklift, constituted a hazard of the employee being thrown from the forklift at the Key West Villas job site. A feasible and useful method, among others, to correct this hazard is to ensure the operator's seat is securely fastened to the forklift per manufacturer's design.
