29 CFR 1910.27(c)(4): The distance from the centerline of rungs, cleats, or steps to the nearest permanent object in back of the ladder was less than 7 inches: Hess Port Reading Refinery - Operators making rounds, performing procedures and responding to emergencies were exposed to fall hazards when using the following fixed ladders: a) FCCU, P-518 Main Column Bottoms Pump, Motor Operated Valve (MOV) Deck - A drain line was located 1.5 inches behind the rung of a ladder. Condition noted on or about 12/15/08. b) FCCU, Flue Gas Cooler, Upper Level Blower Deck - Electrical conduit was located 3.75 inches behind a ladder rung, 10 ft. above the work platform. Condition noted on or about 12/16/08. c) Gas Concentration (Gas Con), T-604 Rich Oil Fractionator, 2nd level - Insulated pipes were located within 2.5-5 inches of the first rung and 1.5-3 inches behind the 7th rung. Condition noted on or about 01/05/09. d) Gas Con, D-623 C3/C4 Splitter Receiver - Insulated lines were located within 3-4 inches of ladder rungs, 13-14 ft. above the ground. Condition noted on or about 01/05/09. e) FCCU, T-501 Main Column, ROB Deck - An insulated pipe was located 1 in. behind the rung of a ladder, 53 in. above the work platform. Condition noted on or about 02/09/09.
