Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to pressurized hoses that were not safeguarded to prevent whipping in the event of hose and/or hose connection failure: a) Warrant 4339 Well 43, Foster Township, McKean County, PA - On or about April 5, 2010, five (5) high pressure hoses were not outfitted with devices to prevent hose line whip in the event of hose and/or hose connection failure. ABATEMENT NOTE: Among other methods, feasible and acceptable means to correct the condition include: 1. Complying with the applicable requirements noted in the International Association of Drilling Contractors (IADC) - Accident Prevention Reference Guide; Section 3.20 High Pressure Lines and Fittings, Item L. 2. Hobble the hose/hose connections with safety straps.
