Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a service rig's guy wires and anchors that were not erected and assembled (rigged up) in accordance with manufacturer specifications and/or industry standards: a) Jefferson County, Warsaw Township, Sprague Wingard 10 well - On or about December 22, 2008, and at times prior thereto --- Employees were exposed to struck-by, crushing, and blunt force trauma hazards from the Walker-Neer Service Rig #4 that commenced well operations without ensuring the rig was erected and assembled (rigged up) in a safe manner according to manufacturer specifications and/or industry standards as evidenced by the improper erection, assembly, and installation of ground anchors: Specifically, these conditions existed on the following ground anchor components: 1. Rear Ground Anchors - The two (2) rear ground anchors had not been set. 2. Forward Ground Anchors - The two (2) forward ground anchors were positioned at improper angles and distances from the service rig. 3. Ground Anchors - The steel stakes were not installed according to any written instructions or pull tested to determine load capacity ABATEMENT NOTE: Among others, one feasible and useful abatement method to correct this hazard is to: (1) Follow the requirements of Walker-Neer Manufacturer Co., Inc. specification sheet which requires four (4) ground anchors connected to crown guy wires. (2) Reference American Petroleum Institute (API) RP4G Recommended Practice for Use and Procedures for Inspection, Maintenance, and Repair of Drilling and Well Servicing Structures, Section 14 Guy Wire Anchors for Portable Masts with Guy Lines (3) Reference American Petroleum Institute (API) RP54 Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, Paragraph 6.3.2 which states that "Well operations involving the rig shall not be commenced until the rig is rigged up in a safe manner." (4) Reference OSHA STD 03-12-003 PUB 8-1.8 Guidelines on the Stability of Well Servicing Derricks (5) Reference OSHA Technical Manual Section IV Chapter 1 Oil Well Derrick Stability: Guy Wire Anchor Systems Abatement Documentation is required for this violation.ion
