Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by and crushing hazards: (a)Nexus Drilling, Rig #4, Daisy Ross No. 2 well site - Employees were performing work on and around the drilling rig. Employees were exposed to the hazard of collapse of the derrick/mast in that the employer did not ensure that the anchor capacities were verified prior to attaching guy lines to the anchors, as determined on 07/25/08. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow accepted safe practices such as but not limited to ; a) verify the capacity of the anchors by pull testing or other method based upon accepted engineering practices that yield equivalent results to pull testing, as specified in the Recommended Practice for Use, and Procedures for Inspection, Maintenance, and Repair of Drilling and Well Servicing Structures (API Recommended Practice 4G, 2004), section 14.5(a), and b) the employer shall be responsible for providing anchorage capable of withstanding the maximum guy line loads, as specified in the Specification for Drilling and Well Servicing (API Specification 4E, 1988) section 3.14.
