valves #4074, #4075, and #4076 that were located on top of the C Module Cold High Pressure Separator. (b)Plant 81 - On or about 02/23/2009, P&ID 81-KF-40272-001, Rev. 12 identified the A Module seal oil system control valve, LV-2294A failing open when the valve would actually fail closed, identified the associated reversing relay, LY-2294A which had been removed, and identified the A Module seal oil reservoir level transmitter and level indicator, LT-1299 and LI-1299 which were actually installed on C Module. (c)Plant 81 - On or about 01/23/2009, P&IDs including but not limited to sheets 81- KF-35164-004, 81-KF-35167-003, 81-KF-35169-003, 81-KF-35169-004, and 81-KF- 35175-002 identified orbit valves with a symbol which was not listed on the P&ID Legend Sheet #187-KD-10000-001. (d)Plant 16 - On or about 03/05/2009, P&ID 16-KF-60532-003, Rev. 18 identified a line tie-in from the pressure relief device, PRD-937 to the relief line, BD522-28"-E when the PRD and tie-in did not exist.
