Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Halliburton Energy Services, Dora Roberts Devonian Unit 3401, Midland County, TX. On 11/21/07, the rated maximum working pressure was exceeded on a wheel valve due to an employee operating and monitoring a HT2000 3512B Pump connected to a manifold truck (Unit #10864618) to set a pump seal. Among others, feasible means of abatement could include: Install a pressure reading device on the high pressure line upstream of the wheel valve so pressure readings can be monitored in the frac van and a global kickout can be activated.
