Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Drilling Rig: Employees were potentially exposed to the hazard of being struck by the derrick in the event of a tip over in that the guy wires stabilizing the derrick were inadequately secured with the use of only two u-bolt clips. Among others, one feasible and acceptable method to correct this hazard would be to follow the API document, Practice 9B, 11th Edition, 2002, titled, "Recommended Practice on Application, Care and Use of Wire Rope for Oilfield Service," Section 3.7.4 which requires the use of at least four wire rope clips when a sheave is in place.
