Text For Citation: 01 Item/Group: 002 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion: At this inspection site, on June 27, 2007, and at times prior thereto, the ground anchor for the auxiliary escape line was not installed. The auxiliary wire rope escape line had been wrapped several times around a compressor unit that was nearby the rig. Among other methods, one feasible and acceptable abatement method involves the employer ensuring the auxiliary emergency escape line system is installed in accordance with American Petroleum Institute Recommended Practice 54, Third Edition, August 1999 Reaffirmed, March 2007, Section 6, Sub-Section 6.10, paragraph 6.10.1 and Geronimo Manufacturing Operators Manual guidelines.
