Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of steam burns due to potential power piping rupture occurrence: At Iatan Station #1 the operating company had not determined the root cause of a steam line rupture in order to develop corrective action to minimize the probability of recurrence in that no consideration was given to factors such as, but not limited to, original pipe design requirements and implied corrosion limits. Among other methods, one feasible and acceptable abatement method to correct the hazardous condition would be to generate a failure report as described in the American Society of Mechanical Engineers (ASME) Code for Pressure Piping B31.1 - 2004 Edition. Abatement certification is required for this item (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of equipment, photographic or video evidence of abatement, or other written records.
