Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards associated with sudden pressure release: On or before April 10, 2007, equipment was installed in an unsafe manner which included the following visible deficiencies on but not limited to the: a.high pressure mud hose from the mud pump to the standpipe was not adequately secured with a retaining device to prevent displacement; and b.the Crosby clips on the wire rope to the tong counterweight on the north side of the substructure were not evenly spaced with an adequate length of dead-end wire rope and one clip had the saddle placed on the dead end of the wire rope. Among other methods, feasible and acceptable abatement methods to correct these hazards would be to: a.Require that all pressurized lines be secured to an adequate safety retaining devices as referenced in the International Association of Drilling Contractor's Accident Prevention Reference Guide. b.Ensure the wire ropes are installed according to the Crosby Clips manufacturers application instructions and guidelines.
