Text For Citation: 01 Item/Group: 002 Hazard: CUTS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to structural collapse hazards: a. On or about June 12, 2007 and at times prior, Nabors Drilling USA, LP as the creating/exposing contractor did not ensure that employees and workers were protected from structural collapse and failure. 1. Strucutral struts designed to support the substructure near the area under the drilling floor were not installed. 2. Structural pins were driven in upside down and secured with welding rods and pieces of pail handles instead of proper cotter keys. 3. Supports for the main mud line were cut off of the structure and rewelded without engineering oversight. Among other methods one feasible and acceptable abatement method would to have the rig erected as it was designed while under the assessment of a structural engineer. Recommendations of the engineer would then be used to train the employees. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
