Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, the mast erection and support pole was bent: a) On or about February 6, 2007 and at times prior, employees were exposed to injuries in that, the hydraulic mast was improperly repaired. Among other methods, one feasible and acceptable method to correct this hazard would be to ensure all repairs are made in accordance with the American Petroleum Institute Recommended Practice 4G Section 6.2.4. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
