Text For Citation: 01 Item/Group: 002 Hazard: UNAPOPPROC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a/ Employees were not prevented from opening the elevator door by the use of a car door restrictor. Car door restrictors had not been installed on any of the elevators. One employee was killed trying to exit the elevator. While ultimate responsibility for correcting the hazards rests with the employer, given his/her superior knowledge of the work place, feasible and acceptable methods of abatement to correct this hazard, among others are: 1. Follow the guidelines of Part II, Section 2.7.4 (a), (b), and (c) of the American Society of Mechanical Engineers Guideline for , ASME A17.3-1999. When a car is outside the unlocking zone, the hoistway doors or car doors shall be so arranged that the hoistway doors or car doors cannot be opened more than 4 inches (102 mm) from inside the car. The unlocking zone shall extend from the landing floor level to a point no greater than 18 in. (457 mm) above or below the landing floor. To meet this guideline elevator car door restrictors must be installed. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS VIOLATION. The documentation should include written verification of abatement, applicable measurements of monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).n
