North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Facility - - Assembly Area: The pedestal-mounted Contrx jib crane with the Jet 1-Ton manual chain hoist did not receive frequent and periodic inspections. b)Facility - - Machine Shop: The pedestal-mounted Gorbel jib crane with the Coffing 1-Ton electric chain hoist did not receive frequent and periodic inspections. REFERENCE:ANSI/ASME B.30.11-1998: Monorails and Underhung Cranes: Chapter 11-2: Inspection, Testing, and Maintenance: Section 11-2.1: Inspection: 11-2.1.1Inspection Classification: a)Initial Inspection. Prior to initial use, all new, re-installed, altered, or modified cranes and monorail systems shall be inspected by a designated person to verify compliance with the applicable provisions of this volume. b)Interval Classification: Inspection procedure for crane and monorail systems in regular service shall be divided into two general classifications based on the intervals at which inspection should be performed. 1)Frequent Inspection: Visual examination by the operator or other designated personnel with records not required, as follows: a)Normal Service - monthly. b)Heavy Service - weekly to monthly. c)Severe Service - daily to weekly. 2)Periodic Inspection: Visual examination by an appointed person making records of apparent external conditions to provide the basis for a continuing evaluation, as follows: a)Normal Service - annually. b)Heavy Service - as in (a), Normal Service, unless external conditions indicate that disassembly should be done to permit detailed inspection, semiannually. c)Severe Service - as in (b), Heavy Service, quarterly. 11-2.1.5Inspection Records: Dated inspection reports or comparable records of critical items, such as hoisting machinery, sheaves, hooks, chains, ropes, wheels, and other lifting devices, as required in paragraph 11-2.1.1(b)(2) shall be maintained. Records should be kept where available to appointed personnel. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to develop and implement an inspection procedure for the crane/hoist systems present and operated within the facility that ensures the required inspections covering the items listed in B.30-11 - Chapter 11-2 and/or the recommendations from the crane/hoist's manufacturers are performed by competent personnel and to maintain documentation of these inspections in a permanent file on-site for the operational life of the crane/hoist systems.ist
